RE: Procedural steps during breast augmentation

First of all you have the discussion with your plastic surgeon during the primary consultation and as many consultations are needed before you are perfectly clear with him as to what your expectations are and how realistically he can provide you the results. These consultations often include probation of various so called sizers that are similar to breast implants in shapes and sizes but they are worn usually under a sports bra to allow the patient to assess the volume and shape of the breast implants.
After you make the appointment for the surgery the surgeon will often consult you once or twice more to measure the chest dimensions and make the preoperative markings on the day of the surgery. It is a usual practice to take photographs of patients during preoperative measurement as a proof of any prior asymmetry. Some clinics like ours include a psychologist and an oncologist consultations before surgery. After the preoperative workup you usually talk to the anesthesiologist. The anesthesia is usually general and its important to specify all your prior medical records as well as allergies or congenital conditions. The surgeons experience counts here immensely as a pocket too small can deform the breast while a large one can cause complications like rotation of implants. At this stage the hemostasis i.e. clogging of bleeding vessels is undertaken aggressively. Presence of bleeders can compromise the final result. Usually drainage tubes are placed during this same process that help to drain the blood out thus preventing pooling postoperative. Usually two or three checks on the status of homeostasis are done after which the surgeon changes his gloves, receives the implants in their sterile containers and places them in their “pockets”. After checking and rechecking their symmetry in many positions the “pockets” are closed usually by some resorbable sutures fixing the new submammary fold. After that the subcutaneous tissues and skin is sutured with intradermic resorbable sutures and bandaged. Sometimes special bandages are required for obtaining a better realignment of tissues. A compressive bandage is then applied. The anesthesiologist takes over and wakes you up. The anesthesiologists supervise you in post anesthesia room for as long as necessary before sending you to your ward. Usually continuous analgesia is administered to avoid postoperative discomfort. Along with analgesics usually antibiotics, hydration fluids and electrolytes plus medication for nausea, dizziness etc may be administered as considered necessary by the doctors. The drains stay usually from between 24-48 hrs and in case of uneventful recovery you are discharged two days after the surgery.

We do this procedure under local anesthesia. You are awake. There is no pain, and total procedure is completed in one hour, and after 2 more hours of stay with us, you can go home.

Under the choosen anaesthesia, the surgeon makes an incision, have diseccion over or under the pectoral major muscle, creating a pocket for the implant, and put the implant inside this pocket. After the surgeon use stichs to close deep and superficial incisions.
