Basically there are 2 surgical approaches there. Intra-oral and sub-mental. Both have their advantages and disadvantages. Then it depends on your main diagnosis, weather you have receding or protruding chin.

During the chin surgery, the chin can be accessed from the oral cavity or externally from the incision under the chin. In our department, we prefer the access from the oral cavity. The incision is placed in the mucous lining. No visible external scars are created.
they may be placed in the mouth in the gingivo-buccal sulcus or in the skin below the chin.
With the intraoral technique, an 1 inch scar will be visible in the lip base, just 4mm above where the lip merge with the jaw. This scar will disappear in less than a month.
Whit the extraoral technique, you will have a permanently scar hidden in the doble chin fold. Not visible at all.
