Otoplasty under local anaesthesia

(Discussion to topic Ear surgery (Otoplasty))

Otoplasty under local anaesthesia

Is a child always required to have general anaesthesia during otoplasty or can children handle the surgery under local anaesthesia? I have a six-year-old son with “dumbo” ears.

Discussion (a thread detail)

Some children may also be operated under local anesthesia, of course, if they are calm and the whole time listening to Surgeon. Most comfortable for the child and the surgeon if the operation is done under general anesthesia or analgosedation.

Prominent ears in children under 12 years old should be performed under general anesthesia as they will not tolerate local anesthesia well. They will not be able to stay still for the procedure that will last about an hour.

I personally always prefer undergoing this surgery under local anaesthesia. The possibility of this in children depends on the child's personality and how he was brought up by his parents and with the psychological preparation before surgery. Usually this is possible at the age of 8 and above as this requires the cooperation of the child, below this age it is very difficult. Usually at this age the child is very motivated to undergo the surgery because of the problems he faces in school by his mates. With some explanation of the procedure and how much pain will he face it is possible. A very important point is to avoid threatening children that if they do something wrong the doctor will give them an injection as a punishment. This statement is very common in some countries and by saying so a barrier is created throughout their lives that they are scared of doctors and in such a case they will never accept a procedure under local anaesthesia. A local anaesthetic cream applied to the area to be injected helps a lot. The surgeon should also be having some nice stories to entertain the child during the surgery.

RE: Otoplasty under local anaesthesia

Dear,
Your son can have the operation under local anesthesia. We have special expertise in this kind of anesthesia, for adults, and children alike. In fact, we very rarely use general anesthesia. We operate children as young as few months old under local anesthesia.

I would like to suggest you to choose a trust and Board Certified Plastic Surgeon. If you have interest in one of us, you must click in our name and write to us.

For adolescents and adults, local anesthesia is possible to make a otoplasty, but we like too much to add an assisted sedation(anesthesiologist by side us and you´ll be monitorized) and talking with us and sleeping and more relaxed. Is safer and less traumatic and less pain.
Child, general anesthesia or venous anesthesia is preferred.

RE: Otoplasty under local anaesthesia

For safety and tranquility of parents, children and the surgical team, this surgical procedure (aesthetic and reconstructive) should always be performed under general anesthesia given and supervised by a physician specialist in anesthesia and resuscitation. Local anesthesia should be accompanied by deep sedation and this is already a risk, if we remember that the child's face is covered with sterile drapes of cloth or waterproof material. If it still child moves or is uncomfortable or low oxygen saturation, we must recur to emergency general anesthesia. Finally, we should strive for the child to always keep in mind a pleasant memory of their experience in the OR.

RE: Otoplasty under local anaesthesia

Like any other operation, otoplasty is an operation, where the patient should not move. Immediate movements of the patient could cause complications. You can not guarantee thet a six-year old child will stay quiet under these stressful conditions in an OR. That is why I would recommend to operate children under general anaesthesia!

kind regards

RE: Otoplasty under local anaesthesia

Ear surgery, or otoplasty, is usually done to set prominent ears back closer to the head or to reduce the size of large ears.

For the most part, the operation is done on children between the ages of four and 14. Ears are almost fully grown by age four, and the earlier the surgery, the less teasing and ridicule the child will have to endure. Ear surgery on adults is also possible, and there are generally no additional risks associated with ear surgery on an older patient.

If you're considering ear surgery for yourself or your child, this information will give you a basic understanding of the procedure-when it can help, how it's performed, and what results you can expect. It can't answer all of your questions, since a lot depends on your individual circumstances. Please be sure to ask your doctor if there is anything you don't understand about the procedure.

Since local anaesthetics carry less risk we prefer to do the procedure with local anaesthetic if the child can tolerate it.

RE: Otoplasty under local anaesthesia

Local anaesthesia is limited for children that are mature enough to tolerate the application of the anaesthetic solution, and that also comprehend that they have to hold the same position for a prolonged period of time, so it is a matter of preoperative evaluation by the attending physician. At 6 years of age it is best to do it under sedation supervised by the anaesthesiologist, and local anaesthesia. Both, the srgical team and the patient are more comfortable and grateful about it.

Yes!
It's possible to perform that surgery (otoplasty) under local anaesthesia and a very soft sedation.
Actually, It is possible also with no sedation depending on the child behavior.

RE: Otoplasty under local anaesthesia

I would normally recommend anyone under about 12-14 years old to have a general anaesthetic for the correction of prominent ears (otoplasty). The operation can take between 60 and 90 minutes, which is a long time for anyone to keep still, let alone a young child. It can also be quite disturbing for children to be able to hear the operation which seems amplified as it is in the ear, as well as the discomfort from the injections of local anaesthetic.
Nowadays general anaesthetics are very safe, so I do not have any hesitation in suggesting one for an otoplasty. On the other hand, most adult otoplasties are performed under local anaesthetic.
