X-HA3 gone wrong

I had a tiny bit of X-HA3 injected above my upper right cheekbone one month ago, and I now have a huge bulge (like a 1 cm. thick worm) starting from the bottom of my eyebrow all the way down my entire cheek. I am petrified! My doctor says he did nothing wrong. I have been massaging it, but it is not going away. In addition, I have a large, puffy bag under my right eye. And just yesterday, I woke up with bulges under my entire right eyebrow. What do I do? Could this be permanent? I am horrified! Please HELP!!! (I live in a third world country, and when a doctor says he won't deal with you, he won't deal with you).

Madam,
that can be an hematoma that will reduce step by step in about 2-3 weeks.
If it is red hot and/or painful it might be an infection that can be easily tretaed with an antibiotic.
It depends also of the amount of product injected. but you said"tiny bit of ...".

Thank you for taking your time to respond to me. I really appreciate it. It is not red, hot or painful, but my massaging and cold compressors seem to be making it worse. If it is a hematoma, what should I do? Also, would a hematoma cause secondary symptoms such as a puffy bag under my eye, and bumps under my eyebrow? Please tell me what to do, as I live in Jordan, and my doctor refuses to help me claiming that he did nothing wrong (and there is nothing I can do about it). If it doesn't start getting better in two to three weeks (because it has been a month now), could this be permanent? I am petrified.

Thank you for your time. The lump appeared immediately. I have been rubbing it and putting cold compressors on it, but it appears to be getting worse. The puffy bag under my eye appeared two days after the injection, and the lumps underneath my eyebrows appeared two days ago. My doctor won't deal with me, as he says he did nothing wrong. What do you mean by, "Reaction?" How do I deal with it? I took antibiotics and antihistamines, but they did not help. Could this be permanent? Please help me.

In case of allergic reactions I have made some good experiences with the local injection of glucocorticosteroids. You should present again at your doctor to exclude haematoma and other `simple` complications and find out if it is an allergic tissue reaction.

Dear Dr. Sinis,
Thank you for your time. My doctor refuses to deal with me, as he says he did nothing wrong (and there is nothing I can do about it). I am petrified! I live in Jordan, and we do not have such injections here. Would steroid-based oral pills help?

Oh I am sorry for this. I dont have any idea nor experience with oral application of corticoids up on this problem. However, I am sure you should find injections also in Jordan. You should find someone, who is a doctor and could estimate the complete situation before you start to take any steroid pills or something like this.

I had several patients requesting to be seen at my clinic from Jordan and Qatar with similar problems.

To answer your question: If the bulge came out straight away or within a few days then it will be bleeding.
If it is several weeks down then line then it could be an infection. Sometimes patients do react to fillers

Did the work look good initially and then deteriorate?

The bulge from my eyebrow down my entire cheek appeared immediately, and I've been massaging it and putting cold compressors on it, but it is not going away and appears to be getting larger. Good Morning,

Be most attending for sings of infections, like fever, pain or redness. In that case you will need to be seing by a doctor. What is your own doctor response? He needs at least give you an explanation of what he thinks is going on. Worst case scenario you will have to wait months until all the effects disappear.
Please remain calm as it is the best thing you can do, and get a doctors opinion upon a personal consultation. Best Wishes.

If it is really HA, try to find a PH with a radiofrequency device; it will get rid of most of the HA also botox if you have some. Useing ultrasound from 1-3mhz will help to improve swelling if present and if you have redness in the area ask your doctor for fluid culture and profilictic broadwide antibiotic.

Dear Nadine,

It can possibly be either a hematoma, if it appeared immediately, as you have mentioned, or it can be an over-injected filler. Infection, and allergic reactions, are also other possibilities. Hematomas usually liquify after 9 to 10 days, and begin to diminish, or they need to be aspirated. You can go to some other doctor, and ask him / her to aspirate, at the site of injection, and apply cold compresses immediately afterwards. If it is a liquified hematoma, or infection, or an over-injected filler, it will subside immediately. I recommend taking a broad spectrum antibiotic, and oral prednisolone tablets, if it does not subside, with aspiration. You should use the antibiotic like Cefadroxil monohydrate 500mg x 2 daily, and Tablet prednidolone 20mg daily x 7 days. You should taper prednisolone after 7 days like 10mg x 3 days, 5 mg x 2 days, 2.5 mg x 1 day, and stop. If either of the above treatments, fail, then we have also seen over-active subcutaneous fibrosis, as a result of liquid fillers. Then you need to contact us in person, for rapid solving of your problem.

Yes, the puffy bag under your eye, and lumps beneath your eyebrow, can also be aspirated, and cold compresses should be applied immediately. Treatment is the same, as I mentioned before.

Dear Nadine: The most important thing right now is calm. Treatment with hyaluronic acid has an antidote of clinical use very effective. HOWEVER, you should have an accurate diagnosis of the situation. Other possibilities such as hematoma or severe inflammatory reaction should be studied as well. Have confidence in your plastic surgeon if he is certified. In these cases always exists a good solution!.

I have had three treatments with hyalauronic acid. The bulge at the side of my face has gone down, but the puffy bag beneath my eye has not. Should I consider more hyalauronic acid beneath the eye, or could I be making it worse?

You should wait and watch for several weeks, if puffy bag beneath your eye, is resolving or not. You certainly should not consider injecting again, which will lead to more puffiness. If it does not resolve after medical treatment, as we mentioned before, then you should contact us again.

Hi

You have to wait, if your doctor put only Hyaluronic acid. Discuss with him about your case. Is he board certified in your/his country? If yes, try to talk with him, calmly.

If you feel pain or fever...maybe NMR(Nuclear Magnetic Ressonance) or ultrassonography(liquids/cysts) can be useful in way to know if you have any haematoma or any anormal reaction or edhema.

If you suspect PMMA/ metacrylate you have to take out if persist petrified way...who will take out?
