I am 8 days post op after a lower facelift.
Now that swelling has gone down, I just noticed the nasolabial line has already come back (although it’s reduced). Worse, when I smile, all the folds I hoped would be gone are still there.
In short, when I smile, I look exactly as I did prior to surgery.

Did my surgeon not pay attention to this area enough?
Or is this just a result of my facial structure?
I have significant vertical maxillary excess, which may be the root cause of my smile looking as it still does.
Curious if I should see my surgeon about this, if any revision is called for?

Hi, you should discuss it with your surgeon. We use a Permanent Smile Lines Filler alongwith face lift surgery, to get rid of the lines permanently. With face lift only, the lines improve, but dont go away completely.

Hello :) During the face lift, the skin is pulled to smooth out wrinkles and to some extent that can help with nano labial folds. But to what extent, it depends on patient's facial structure and anatomical conditions . Sometimes for example fat transfer to nano labial folds can be used to treat that area. Please note that it's still too early to judge the results yet :) It's best to contact the surgeon who performed the surgery regarding advice.

Thank you both. I have a follow up next week.
I don’t mind some lines, but there is sagging and folds which were the primary reason for the surgery.
If I pull the skin just a tiny amount there is an instant significant improvement.
Which tells me that more focus lifting this area would have led to a much better result.
I am nervous

Hi, since you had a lower face lift, whole of your naso-labial lines, have not been covered by surgery, only the lower parts of these lines have been covered. We do not suggest fat for these lines, since in our extensive over 3 decades of experience with fat transfer, fat disappears very quickly from the smile lines. We use a permanent smile lines / wrinkles filler for these and other lines and wrinkles. You should discuss it with your surgeon in the upcoming follow up meeting.
