On the basis of the photo, you qualify for Gynecomastia procedure and glands can be removed. However, another problem is saggy skin which definitely will be left after that procedure. Breast Lift will be necessary as well to get rid of that but after 4/5 months.

It is possible to have a completely Flat chest. However, in your case, since there seems to be sagging and excess skin you would require a breast lift as well.
