My eyelids dropped with the age and i looked few years older then i was. I found your clinic online and i was very impressed with the service. The surgery was performed by dr. Vasek and honestly it has been couple of months and my face has much younger look. Would recommend your clinic.

Had lower blepharoplasty done at the Forme clinic this week. Results look Good so far. Procedure was painless and Dr Vasek went through it in detail in the consultation, very well put over and explained to me. When I asked for another procedure ( fillers ) he said no not a good idea. Not there to grab your money and perform anything you ask for - which I think is good as there are providers out there that will gladly do this. Reception efficient and friendly , would definitely recommend. The aftercare pack is extensive and well put together with instructions etc as are the verbal instructions, all very efficient which makes you feel like you are dealing with professionals.

I picked Formé Clinic and specifically Dr. Vašek because my friends recommended it. I had a blepharoplasty on upper and lower eyelids and I was really satisfied with result from the beginning. Dr. Vašek and his team have very profesional approach. From the first communication through emails (their responds were always very quick) to the post surgery care i knew i was in good hands. I really recommend Formé clinic! My personal experience with my face lift+ Upper eyelid surgery in this clinic is superb. You pay the best price and you are in the professional hands. The doctor takes his time and answers each and every question before your surgery. The whole staff, including Physician are very polite and always there for you. The clinic is very clean with comfortable rooms. One gets options to select his , her food menu during his her stay in the clinic. Overall, Excellent.

I had a great experience at Forme, for the price of my surgery I highly doubt I could get anywhere near the same level of quality and care without spending thousands money at another clinic. The staff were helpful and comforting and the operation carried out by Dr Vasek was faultless, I highly recommend going to Forme you won't be disappointed.

I have written a review because I am grateful to the Forme team but also because of my experience with Graves disease. A number of factors led me to access treatment abroad and not just the difference in cost which is significant. Having had two consultations in UK I felt that there was a reluctance (not a refusal) to treat me because I have graves disease – which I did not find encouraging. Around the time I was first considering Blepharoplasty I can remember stepping into a mirrored lift on the way to a meeting and feeling shocked by my appearance. I had experienced a series of significant bereavements and it felt as though the grieving had left its mark across my face. How much the graves disease contributed to my fatty eyebags I don’t know but I looked beaten up and like I had a permanent black eye. I delayed access treatment for another year to ensure there were no developments with my eyes and now I am very pleased with my decision to choose Forme Clinic. If I was to complete a cost benefit analysis I would say that having a reason to travel alone to Prague was a huge bonus. It is the most amazing place and the architecture is awe-inspiring. I didn’t experience any anxiety until just before the surgery because I was positively distracted by the environment. The clinic is very efficient, providing for me a unique medical experience with no delay or faffing around. The staff gave me half a tablet prior to the surgery which did concern me because I realised that I would be aware throughout the procedure. Dr Vasek said that sedation was unnecessary and on reflection I would agree with him. When the local anaesthetic was injected I felt my eyes become numb and heavy as they closed. They stayed closed throughout the procedure. I could only see the bright light through my eye lids and feelings of pressure but no pain. It felt as though someone was tugging on my eyelashes. I can remember being surprised by how quickly the first lid was done and not feeling worried about the other three because it was OK. All four lids took less than an hour. After the treatment I was glad that I had not been sedated as I was alert enough to go out and see more of Prague. I had a session with frozen peas (which are more supple than the gel masks the clinic give you) then went to visit a nuclear bunker. The whole experience was entirely positive. Dr Vasek is a confident and skilled surgeon and the clinic staff ensure that it runs efficiently. There was no need to take any painkillers at all afterwards and I would say the worst thing was the itching between days 2-5. This did torment me to the point I was making fists with my toes and I was worried about scratching my eyes during the night. I did not expect to heal so quickly and have had no problems at all with my eyes / lids and have been able to read, use computers etc. I look more alert, less annoyed and have no eye bags. I expected some improvement to my eyes but did not expect symmetry because my eyes were different to start with. I noticed after the surgery that the incisions around my eyes had been made differently and now my eyes look the same. Dr Vasek must have factored in the eye difference - I think that he is amazing. I had upper and lower eyelid surgery on the 1st September at forme clinic in Prague ..the surgeon was Dr Vasek. From my initial contact via email to the actual day of the surgery they were brilliant, always replied to emails straight away. Very helpful with any questions I had and even arranged a free transfer from the airport to my hotel. The surgery is clean and modern and in a perfectly safe and central location. I travelled on my own and it was absolutely fine. The surgery took and hour and was totally pain-free. After I left I had quite a bad bleed from my right eye and had to go back but the girl on reception was brilliant ans called the doctor and the nurse straight back . It was fine and nothing out of the ordinary but they reassured me and it was no problem at all. It's two months on now and the results are improving every day. I'm so happy and would highly recommend them to anyone. I'm sure I'll be back

I arrived at forme clinic on the 5th Oct, had full range of tests to ensure I was in good health prior to surgery. I had a consultation with both Dr Vasek and Dr Benes for two different procedures to be preformed in one single operation. Dr Vasek was adamant that I did not need eye lid surgeries and that it would be a waste of money due to very tiny correction that could be achieved at present and that I was too young in his experienced opinion. In my consultation with Dr Benes he told me that I would ideally benefit from more liposuction to a further 2 areas to achieve a better results. In my original quote I was only advised to have 1 area liposuctioned, therefore paying for an additional 2 areas would of been difficult had I still goneed ahead with eyelid surgery. Thankfully the money I saved on not having eyelid surgery covered the cost of the extra lipo. It was a more costly abominalplasty plus lipo surgery compaired to some of the other ladies at the clinic I had talked to but I believe this was due to my tummy carrying substantial excess fat and a more lengthy procedure. Once out of surgery, I was cared for by the clinic nurses who were extremely professional and very caring ladies, nothing was too much for them and they ensured that I was well looked after. Dr Benes came to see me twice and explained how to proceed with aftercare and such on return to the UK. Dr Benes manner was approachable and friendly, he answered every question with patience. Finally, I was discharged on the 7th Oct with a full aftercare pack including: antibiotics, anti swelling tablets, pain killers, dressings, steri strIps and some medication to help boost my immune system and aid healing. I was given thorough explanations from Nurse about when to take Meds and when to change dressings. I also received typed instructions too. The clinic provided a taxi to cover all transfers needed which really was a huge help. All surgery, medications, food and drinks at clinic, nursing care, aftercare pack, compression garment and transfers were included in the price. I'm currently on day 5 after surgery and swelling has hit full force but despite this I am extremely happy with the results. I hope to go back next year for a breast lift!

Totally worth it. My surgeon was Dr. Vasek. He is so kind. Clinic is really nice place in the center of the city. Everything was modern and clean and their care was superb. Definitely would use Forme clinic again.

I had a facelift done by MUDr Petr Jan Vasek at the Forme Clinic in Prague, Czech Rep. He is a great surgeon - totally professional and highly skilled. The results he achieves are very natural and a real improvement. I wouldn't hesitate to recommend him to anyone who wants this procedure done.
